- Reporting patient status changes and collaborating with the care team.
- Conducting psychosocial assessments and developing discharge plans.
- Documenting patient care accurately.
- Educating patients and caregivers about resources.
- Associate's Degree or Diploma in Practical Nursing.
- 3 years of acute or long-term care experience.
- 1 year of home health experience preferred.
- Valid LPN license, CPR certification, driver's license, and auto insurance required.
