Home Health LPN

We are seeking a Home Health LPN to provide compassionate, home-based patient care under the direction of a registered nurse. Responsibilities include implementing nursing care plans, reporting patient status changes, conducting psychosocial assessments, and collaborating with the care team to develop discharge plans. Candidates must have an Associate's Degree or Diploma from an accredited LPN program, 3 years of acute or long-term care experience, and a valid LPN license. A sign-on bonus is available, with a starting pay of $22.00/hour. Travel is required up to 75%. Comprehensive benefits are offered.
