Position Overview
We are looking for candidates with a minimum of 1 years of nursing experience. Those with prior HOME HEALTH EXPERIENCE will be given preference.
Responsibilities
- Serve as the patient’s advocate and maintain ongoing communication with physician, referral sources, caregivers and facility management to facilitate coordination of care
- Responsible for timely completion and synchronization of visits within the patient home via device (EMR)
- Contributes to the assessment of the health status of individuals or groups.
- Participates in the development and modification of the strategy of care
- Implements the appropriate aspects of the strategy of care and participates in the evaluation of responses to the strategy of care.
