Job Position: Health Coordinator ll (aka Support Coordinator ll)
Location: Remote - work from home (must reside in IL)
Pay Rate: $23.00/hr.
Shift: M-F 8 AM to 5 PM
Description: Supports administrative care management activities including performing outreach, answering inbound calls, and scheduling services. Serves as a point of contact to members, providers, and staff to resolve issues and documents member records in accordance with current state and regulatory guidelines.
Responsibilities:
Outreach members telephonically.
Answer incoming calls from members or clients.
Create Case, Create Tasks, Create Referrals in member’s chart.
Complete screenings with members.
Filter Excel Spreadsheet
Utilize Outlook Calendar for daily planning.
Document member accounts.
Assist Member in downloading necessary App and Onboarding.
Educating members on Management and client systems.
Education/Experience:
Requires a High School diploma or GED
Requires 1 – 2 years of related experience.
Provides outreach to members via phone to support with care plan next steps, community or health plan resources, questions or concerns related to scheduling and ongoing education for both the member and provider throughout care/service.
