Job Position : Bilingual Care Manager (RN / LSW)
Location : Chicago, IL 60608
Pay Rate : $30.00 / hr - 40 / hr (based on experience)
Availability between : 8 : 30AM-5 PM or 9AM - 5 : 30 PM (M-F), and evenings one Wednesdays until 8 PM.
Bilingual - Spanish (Must be fluent in Spanish)
Job Position : Licensed Case Manager (RN)
Position Purpose : Perform care management duties to assess, plan and coordinate all aspects of medical and supporting services .
Job Duties :
- Develop, assess and adjust, as necessary, the care plan and promote desired outcome
- Assess member's current health status, review past and present treatment plan and services
- Coordinate services between Primary Care Physician (PCP), specialists, medical providers, etc.
- Develop plan of care based upon assessment with specific objectives, goals and interventions
- Facilitate member access to community based services
- Monitor referrals made to community based organizations, medical care and other services
- Enter and maintain assessments, authorizations, and pertinent clinical information into various medical management systems
