Job description
A Blended Case Manager (BCM) provides blended case services to adults and children who suffer from a serious mental illness or emotional disturbance. The BCM staff will ensure consumers are linked to necessary resources in order for the consumer to remain in the least restrictive and most normal setting. The BCM staff promotes independence so that the consumer will be educated on resources that will encourage appropriate knowledge of resources in the community in which they live. The focus of the Alternative Community Resource Program (ACRP) BCM services are recovery oriented. Blended case management services are consumer lead and the consumer determines their own path of recovery by maximizing independence and self-sufficiency. The BCM staff utilizes an Environmental Matrix (EM) testing tool to evaluate the necessary level of service and the EM test will be updated at least every 6 months. In addition, the individual and family input and input from other health providers involved in the care; number of crisis contacts; current or anticipated stressors will be obtained during the BCM intake. An assigned BCM staff will work with the individual and families to access, link, coordinate and monitor needed services as well as provide support and encouragement.

Requirements:
- A bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other social sciences, criminal justice, theology, nursing, counseling or education, or;
- Be a registered nurse, or;
- A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology or other social science and 2 years' experience in direct contact with mental health consumers, or;
- A high school diploma and 5 years of mental health direct care experience in public or private human services with employment as an intensive case management staff person prior to April 1, 1989.

Responsibilities
- Conduct comprehensive assessments of clients' needs and develop individualized care plans.
- Provide crisis intervention and addiction counseling to support clients in high-stress situations.
- Facilitate access to behavioral health services and resources, including Medicare and Section 8 housing.
- Mentor and supervise junior staff, ensuring adherence to best practices in social work.
- Engage in motivational interviewing to empower clients towards positive change and workforce development.
- Collaborate with community organizations to enhance child welfare and support systems.

Pay: $17.15 - $18.69 per hour
