What are the responsibilities and job description for the Lead Health Navigator position at Alternative Community Resource Programs Inc?
Job Summary:
Under the direction of the BCM Director, the Lead Health Navigator will be responsible for collaborating with members of the treatment team to arrange for all necessary medical, rehabilitative, and social services for the client. In addition, the Lead Health Navigator will be responsible for monitoring and arranging changes in the medical needs of the client.
Duties/Responsibilities:
Attend weekly BCM supervision meetings and report on current client issues, updates, changes to the program, and any other important deadlines.
Attend quarterly BHHP QIT meetings and discuss changes/important matters with the wellness team.
Complete Physical Health Assessments for the wellness program.
Complete the Lead Health Navigator planning tool with each new wellness client and update the document yearly.
Correspond with the client's PCP/Specialty Physicians for coordination of care.
Develop preventative wellness plans for clients new to the system to preclude the development of chronic health conditions.
Gather and record client information including but not limited to vital signs, height, weight, and waist circumference, and pursue more specific investigations as needed; Identify abnormal diagnostic data and/ or concerns, and report information to providers.
Maintain the BHHP spreadsheet and tracking document.
Plan, schedule, and conduct meetings when necessary; Attend all mandatory meetings.
Respond to phone calls, voicemails, emails, faxes, etc.
Review the CCBH BHHP registry monthly and discuss any problems/changes with the wellness team.
Scanning and attaching documents, including but not limited to the following: prescriptions, letters for clients, correspondence with the client and/or a party involving a client, etc.
Minimum Job Requirements:
Educational Requirements:
Possess a Master's Degree in Psychology, Clinical Mental Health Counseling, or another closely related field and possess a valid LPC, LSW, or LCSW license in the state of Pennsylvania.

Possess a Bachelor's Degree in Nursing and have an active PA License as a Registered Nurse.

Possess a Bachelor's Degree in Psychology or a health-related field.
