Job Summary
The Clinical Supervisor oversees all clinical quality components of home health agency services. Responsible for ensuring compliance with regulations, accreditation standards and agency policies and procedures. Possesses strong communication, management, and customer service skills to promote the agency’s mission and organizational culture.
Responsibilities and Duties
- Make patient and personnel assignments
- Coordinate patient care
- Assure that patient needs are continually assessed
- Assures the development, implementation, and updates of individualized plan of care.
- Act as a liaison between Home Health agency and patients, medical facilities, and physicians.
- Supervises and ensure training for field team of RNs, LPNs, and home health aides.
- Ensure clinical quality assurance
- Ensure all programs and clinical operations are in full compliance with state and federal regulations, including HIPAA.
- Promote agency’s mission and organizational culture of providing the highest quality of care standards.
Qualifications
- Graduate from an accredited school of nursing; Registered Nurse
- RN licensure in good standing in the Illinois; current CPR.
- Minimum 2 years acute care experience; Minimum 2 years home health or long term care experience; 2 years supervisory/ management experience preferred
- Knowledge of OASIS protocol
- Strong clinical judgment and critical thinking skills
- Strong organization and prioritization skills
- Exceptional communication skills
- Adaptable to any situation and able to concentrate under stress and tension
- Knowledge of Medicare guidelines
- Experience and knowledge of Joint Commission on Accreditation of Healthcare Organizations (JCAHO) and/or Community Health Accreditation Partner (CHAP) Accreditation
- Knowledge of quality assurance protocols.
- Proficiency with computer-based patient database systems.
- Proficiency with Windows OS including typing, email, internet, and Microsoft Office
