Services we provide consist of home and community-based programs and in-home supports for those living independently.Our main office is in Bow, NH. All individual needs are unique and growth can only occur at a pace an individual is capable of. APT combines a number of modalities when working with individuals.
APT always maintains the flexibility to match and meet individual needs.
APT will actively seek alternative solutions for an individual should the current placement or treatment become ineffective or stagnant.
APT strongly believes in the importance of matching individuals to homes with others who have similar skills, abilities, hobbies, and interests. Home Care Providers (HCP) are well-vetted and receive ongoing clinical support and home visits as they assist an individual in obtaining their personal goals and integrating into their community.
Respite services are provided upon request. HCPs are able to utilize only trained staff for the purpose of respite.
Role Description
This is a full-time, on-site role for a Direct Support Professional located in Bow, NH. The Direct Support Professional will assist individuals with intellectual disabilities in their daily activities, ensuring their safety and well-being. Responsibilities include providing personal care, implementing individual support plans, assisting with meal preparation and medication administration, and participating in community activities. The role involves close collaboration with healthcare professionals and family members to support each individual's needs and goals.
Qualifications
- Experience in Human Services and working with individuals with Intellectual Disabilities
- Strong Interpersonal Skills and effective Communication abilities
- Willingness to undergo Training and learn new skills related to direct support
- Compassionate and patient demeanor
- High school diploma or equivalent; relevant certifications a plus
