Alternatives (for the Older Adult, Inc.) is a 501(c)(3}, private, non-profit organization serving sixteen counties in Midwestern Illinois. Since 1984, it has been the mission of Alternatives to promote the independence and quality of life for older adults, adults with disabilities and their families. Alternatives' professionals are connecting with ONE IN FIVE individuals over the age of 60 in a sixteen-county region.
BASIC PURPOSE/SCOPE:
To receive, investigate and resolve complaints made by or on behalf of individuals age 18 and older who are residents of Long-Term Care Facilities. To provide support to family or friends of older adults requiring care.
PRINCIPLE RESPONSIBILITIES:
1. Implementation of the Long-Term Care Ombudsman Program.
Responds to all referrals in accordance with program standards. Investigates and resolves complaints involving residents of licensed long term care facilities as assigned.
2. Resident Rights.
Visit and Inform residents of long-term care, assisted and supportive living facilities of their rights and advocates for resident rights.
3. Documentation.
Completes all documentation in a timely manner and in accordance with program standards. Utilizes the Peer Place system appropriately for documentation.
4. Education and Information.
Provides information to residents or the public on long term care, the rights of residents and available services, including the Ombudsman Program. Assists Regional Ombudsman in providing public information and education.
5. Networking.
Maintains a close relationship with other organizations and facilitates referrals to address resident or caregiver concerns to other agencies.
6. Caregiver Support
Provide individual support and follow up to assigned caregivers at least quarterly. Assure individualized challenges are identified and matched to relevant support or resources available.
JOB REQUIREMENTS:
Bachelor’s Degree required. Must have achieved the equivalent of three (3) years of full-time work experience with at least two years in advocacy, aging, social services, health care or related fields. The ideal candidate must have strong communication, documentation and counseling skills. This is a 24 hour a week position.
