Job Summary:
The Clinical Care Specialist - Patient Support is a vital member of our healthcare team, responsible for delivering high-quality patient care and support. In this role, you will perform comprehensive assessments, ensure accurate health records, and identify individualized care plans.
Key Responsibilities:
- Assist registered nurses in identifying patient needs, prioritizing care, and setting goals based on patient characteristics.
- Contribute to the evaluation of care plans by gathering, observing, recording, and communicating patient responses to interventions.
- Demonstrate competency and ability to carry out department-specific functions according to standards of care, policies, and procedures.
- Function as a member of the multidisciplinary team, contributing to the implementation of an integrated patient-centered care plan.
- Assign or delegate responsibility for performance of nursing interventions to others with necessary skills and competence.
- Identify early warning signs of a change in patient condition and report accordingly.
Certifications and Requirements:
- Valid Licensed Practical Nurse (LPN) certification from the Minnesota Board of Nursing
- Basic Life Support (BLS) certification from the American Heart Association
Work Environment:
This position requires frequent standing, walking, stooping, bending, reaching, and handling objects. The ideal candidate will be able to lift up to 20-40 pounds and work in an indoor environment with occasional exposure to moderate temperatures.
Driving Requirement:
This position is classified as an infrequent driver, requiring minimal driving responsibilities.
