Key Responsibilities
- Assist registered nurses and licensed practitioners in identifying patient needs, prioritizing care, and setting goals based on age, ethnic origin, economic status, lifestyle, creed, or philosophical beliefs.
- Contribute to the evaluation of the plan of care by gathering, observing, recording, and communicating patient responses to nursing interventions.
- Modify the plan of care in collaboration with licensed practitioners.
Requirements
- Demonstrated competency and ability to carry out department-specific functions according to department standards, policies, and procedures.
- Functions as a member of the multidisciplinary team, contributing to the implementation of an integrated patient-centered care plan.
- Identifies early warning signs of a change in a patient's condition and reports appropriately.
- Collaborates and communicates relevant and timely patient information with patients/families and other multidisciplinary team members.
Certifications and Education
- Hold a valid LPN license from the Minnesota Board of Nursing prior to start date.
- Obtain Basic Life Support (BLS) certification from the American Heart Association within 90 days of start date.
Working Conditions
- Work indoors with occasional exposure to extreme temperatures.
- Perform duties that require frequent standing, walking, stooping, bending, reaching, and handling/finger/feel activities.
