Job Summary
Altru Health is looking for a compassionate and detail-oriented Patient Advocate Coordinator to support our healthcare team in delivering exceptional patient care.
Key Responsibilities:
- Work collaboratively with registered nurses and other licensed practitioners to identify patient needs, prioritize care, and set goals.
- Contribute to the development and implementation of care plans that meet individual patient needs.
- Demonstrate expertise in performing department-specific functions and adhering to established standards of care, policies, and procedures.
- Report any changes in patient condition promptly.
Qualifications:
- Licensed Practical Nurse (LPN) certification from the Minnesota Board of Nursing prior to start date.
- Basic Life Support (BLS) certification from the American Heart Association within 90 days of start date.
Working Conditions:
This role requires frequent standing, walking, stooping, bending, reaching, and handling/finger/feel activities. The employee must be able to lift up to 40 pounds and push/pull medium-weight objects.
