**Key Responsibilities **
- Provide health care services in clients' homes
- Perform domestic and household tasks
- Transport clients to appointments and errands
- Monitor clients’ vital signs and report their condition using the WellSky App
- Assist with personal care activities
- Maintain client care records and document services provided
- Offer companionship and emotional support
**Qualifications **
- Home Health Aide (HHA) or Certified Nursing Assistant (CNA) certification
- Proven experience in a home health setting
- Level 2 background screening
- Current CPR certification
- Current TB test (within the past year)
- Valid driver’s license and reliable transportation
**Position Details**
- Locations: Jacksonville, FL (Clay, Duval, and St. Johns Counties)
- Job Types: Part time to Full Time
- Salary: $16.00 - $20.00 per hour
