Required Documents
1. HHA Certificate (minimum 40 Hours and please provide syllabus) or current CNA License
2. Current CPR Certification (American Heart Association, Red Cross, or Health & Safety Institute). Online CPR certification not accepted.
3. CEU Certificates: HIV/AIDS, Domestic Violence, Assistance with Self Meds (2hr minimum), Alzheimer/Dementia (3hr minimum).
4. Permanent Resident ID or Work Authorization ID (if applicable)
5. Professional Certificate of Insurance with Liability Coverage
6. Social Security Card
8. Current Driver’s License or ID Card
9. Current Car Insurance (if applicable)
10. Physical Exam within 5 months old or less. Must state you FREE OF COMMUNICABLE DISEASE.
11. COVID Vaccination Card (collected if available, but not required)
