Role Description
This is a part-time on-site role for a Private Duty LPN located in Melville, NY. The Private Duty LPN will provide high-quality nursing care according to individualized care plans. Responsibilities include administering medication, monitoring vital signs, providing wound care, and assisting with daily living activities. The LPN will also communicate effectively with patients, families, and healthcare providers to ensure continuity of care and maintain accurate patient records.

💰 Pay Rate: $35/hour
📆 Schedule: Mon-Sunday 3pm-11pm / Wed, Thur-Sunday 7am-3pm / Friday-Saturday 11pm-7am
Patient Needs:
- C -5 Quad
- 1 person assist
- BP monitoring
- Help w daily activities
- No hoyer lift
Qualifications/Education:
- At least 1 year of LPN experience in a Home Setting
- LPN License and Registration
- NPI #
- Malpractice Insurance
- BLS from American Heart Association
- Physical, QuantiFERON within a year
- MMR titers
Responsibilities:
- Perform patient care duties and treatments as per care plan.
- Provide health promotion, counseling, and education.
- Administer medications and other personalized interventions.
- Coordinate care, in collaboration with a wide array of health care professionals.
- Document nursing notes regarding patient care daily.
- Must be able to lift 50lbs.
- Must be able to stand/work for extended periods
- Additional responsibilities as required.
