Looking for a licensed CSW or LSW to join our friendly staff in providing exceptional caring services at an Adult Medical Day Care Center.
Experience: No prior experience needed
- Providing initial assessments and developing care plans based on the individual needs of the clients
- Assisting in the admission process, authorizations of new clients, re-authorizations etc.
- Assisting clients in applying for appropriate social services
- Complete quarterly and annual reviews on clients
- Develop support groups and educational programs for caregivers and clients.
