Looking for a full-time Social Worker in Ocean County for an Adult Medical Day Center. The Social Worker is responsible for assessing, counseling, intervening, and educating the clients as well as their family members.

Responsibilities:
- Providing initial assessments and developing care plans based on the individual needs of the clients
- Assisting in the admission process, authorizations of new clients, re-authorizations etc.
- Assisting clients in applying for appropriate social services
- Complete quarterly and annual reviews on clients
- Develop support groups and educational programs for caregivers and clients.
Qualifications:
- Must be certified by the New Jersey State Board of Social Work Examiners (CSW)
