Proof of vaccination must include the vaccine type, lot number, expiration date, date and site administered . The full name of the employee must also be visible in the submitted record. If the vaccines were administered when the employee was a child and is applying with different name, (married, etc.) then this must be indicated in the documentation.
As an alternative titers from a diagnostic lab indicating appropriate level of immunity can be submitted.
