Primary Care clinic for Critical Care Access hospital in bush Alaska. Must have high skill level for flexibility, communication skills, cultural sensitivity. Prefer LPN with OB clinical experience.

Proof of vaccination must include the vaccine type, lot number, expiration date, date and site administered . The full name of the employee must also be visible in the submitted record. If the vaccines were administered when the employee was a child and is applying with different name, (married, etc.) then this must be indicated in the documentation. As an alternative titers from a diagnostic lab indicating appropriate level of immunity can be submitted.
