Key Responsibilities:
- Develop and implement individualized patient care plans that meet the unique needs of each patient.
- Provide direct patient care, including observation, assessment, and health teaching.
- Collaborate with other healthcare professionals to ensure effective coordination of patient care.
- Conduct regular assessments and reassessments to update patient care plans as needed.
- Document patient care and progress in accordance with Agency procedures.
Requirements:
- Current Nursing licensure in your state of employment.
- Graduate of an approved School of Nursing.
- Minimum of six (6) months recent work experience as a home care nurse or equivalent experience in a related field.
