Job Title : Licensed Vocational Nurse / Licensed Practical Nurse (LVN / LPN) - Home Health

Job Specialty : Home Health

Job Duration : 13 weeks

Shift : Day Shift 5x8-Hour (08 : 00 - 17 : 00)

Guaranteed Hours : No

Experience : Minimum 1 full year of Adult Home Health or Hospice experience

License : Active LVN / LPN license

Certifications : CPR, BLS

Must-Have :

Proficiency in HomeCare HomeBase (HCHB)
Ability to travel up to 75 miles from the home clinic
Reliable transportation, driver's license, and liability insurance

Job Description :

Provide, coordinate, and direct home nursing care based on physician's orders and agency policies

Utilize the nursing process effectively to ensure high-quality care

Supervise and evaluate Home Health Aides and other nursing staff

Conduct home visits and attend periodic office meetings as required

Handle various types of visits such as Start of Care, Wound Care, Follow-up Visits, etc.

Maintain proficiency in documentation using HomeCare HomeBase (HCHB)

Adhere to productivity expectations and complete all documentation within 24 hours of appointments

Must be open to driving to patients' homes daily and weekly clinic meetings

Accept all assigned patients, regardless of distance, and complete all visits within the given time frame
