Job Title : Physical Therapist Assistant (PTA) Home Health

Job Specialty : Therapy / Rehabilitation

Job Duration : 13 Weeks

Shift : Day 5x8-Hour (08 : 00 - 17 : 00), Monday - Friday

Guaranteed Hours : No guaranteed hours

Experience : Minimum 1 full year of Adult Home Health experience

License : Must have license in hand

Certifications : CPR, BLS

Must-Have :

Reliable transportation
Valid driver's license
Agency-required liability insurance
Proficiency in HomeCare HomeBase EMR

Job Description :

Conduct home visits and attend periodic office meetings

Manage an average of 5-6 patients per day

Earn approximately 6-8 points per day based on patient visits and activities

Travel up to 75 miles from the home clinic, with mileage reimbursed at the IRS standard rate

Cover the Worcester / Marlborough territory

Complete documentation within 24 hours of a patient appointment

Maintain flexibility regarding patient scheduling and travel distances

Participate in the weekly clinic meetings and complete all required documentation

This is a home health assignment requiring daily travel to patients' homes
