We are looking for a qualified Traveling Adult Healthcare Professional to join our team at Amedisys Home Health Care. As a traveling nurse, you will have the opportunity to work with diverse patients and families, providing individualized care and support in their homes.
Key Responsibilities:
- Provide comprehensive home health care services to patients with complex medical conditions
- Develop and implement effective care plans that address patients' physical, emotional, and social needs
- Collaborate with healthcare professionals to ensure seamless care coordination and optimize patient outcomes
- Document patient interactions and care provided in accordance with regulatory requirements
- Participate in ongoing education and training to stay current with best practices and industry developments
Essential Qualifications:
- Minimum 1 full year of adult home health experience or 1 full year of hospice experience
- Recent specialty experience required
- Current BLS certification
- Ability to travel up to 75 miles from home clinic
- Proficiency in HomeCare HomeBase (HCHB) and Oasis software
