Maintains full caseload, complete with ongoing individual service plansAssess ongoing changes in behavior, circumstances, or conditions that may affect client safetyEngages / involves client and sponsor in the casework processPrepares, compiles, and submits accurate case filesServes as a liaison with stakeholders (i.e.. legal providers, immigration court)Actively participates in seminars, workshops, educational programsCompletes home visits to meet with sponsor (if applicable)Maintains contact with clients through phone calls and home visits (if applicable)Consults with supervisors to determine decisions as they relate to the safety, well-being and permanency of patientDemonstrates client progress toward risk reduction, achievement of treatment goals and positive case outcomesMaintains accurate records, files, forms, statistics and additional relevant information in accord with agency policy, licensing, and / or funding requirementsConducts in-home assessments (if applicable)Performs other duties as assigned / necessaryMinimum Requirements :
Bachelors Degree in Behavioral Sciences, Human Services, or Social Services fields REQUIREDExperience in Case Management, Behavioral Sciences, Human Services or Social Services preferredComplies with all relevant professional standards of practiceParticipation and completion of Amergis' Competency program when applicableCurrent CPR if applicableTB questionnaire, PPD or chest x-ray if applicableMust meet all federal, state and local requirementsSuccessful completion of new hire training as applicable to job siteUnderstand patient confidentiality and HIPAA requirementsAbility to effectively elicit / provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is requiredComputer proficiency requiredMust be at least 18 years of ageBenefits
