Job Responsibilities:
- Follow Client’s Individual Service Plans (ISPs): You’ll be responsible for ensuring that each client’s unique needs are met by adhering to their personalized care plans.
- Companionship: Provide emotional and social support to clients, encouraging positive interaction and relationship-building.
- Personal Care Assistance: Assist clients with daily living activities such as bathing, grooming, dressing, and other personal care tasks.
- Community Involvement: Encourage and facilitate clients’ participation in social, recreational, and community activities to foster independence and integration.
- Transportation: Drive clients to appointments, social outings, and community events, ensuring safe travel to and from locations.
Qualifications:
- Must have a valid driver’s license and reliable vehicle.
