Director of Social Services-Nursing Home
Located in Huntsville, TN
Salary: $50K - $60K; Depending on experience
Qualifications:
- Must have BSW or MSW
- Must have experience as a Director of Social Services in an LTC/ nursing home setting
- Must have some knowledge and understanding of PDPM
- Must have strong organizational and case management abilities
- Strong leadership and management skills
- Strong verbal and written communication skills
Job Responsibilities:
- Responsible for coordinating all admissions and discharges to ensure the facility census is kept at its maximum capacity.
- Conducting initial social and emotional assessments of new residents to understand their needs, support systems, and desired level of involvement in activities.
- Regularly communicating with residents' families, providing updates on their loved one's well-being, addressing concerns, and facilitating family meetings.
- Identifying and connecting residents to appropriate community resources such as support groups, volunteer services, and transportation options
- Organizing and facilitating social activities and events that cater to diverse resident interests, promoting socialization and engagement
- Acting as an advocate for residents by ensuring their needs are met and addressing any concerns related to their social or emotional well-being
- Assisting with discharge planning by coordinating with healthcare providers, family members, and community services to ensure a smooth transition upon leaving the facility
- Participates in an interdisciplinary team approach to better serve the needs of the facility population.
