VP of MDS / Case Management – Skilled Nursing Facilities

Candidates MUST be based out of Lakewood, NJ or Cleveland, OH

Salary : $110K to $160K range

Qualifications :

  • Must have valid RN license
  • MUST have current / recent regional MDS experience in skilled nursing facilities / nursing homes
  • Must have recent nursing home experience
  • Must be willing to travel
  • Extensive knowledge of Medicare reimbursement system, compliance and eligibility
  • Must have extensive MDS experience in long term care
  • Knowledge of Resource Utilization Groups (RUGs), PDPM, Insurance and Case Mix
  • Must be willing to travel
  • RAC Certification is a plus
  • Must have strong leadership skills
  • Strong written and verbal communication skills

Responsibilities :

  • Oversee several skilled nursing facilities to ensure accurate MDS completion.
  • Coordinate MDS / RAI within a transdisciplinary approach to ensure the highest degree of quality care is maintained at all times.
  • Assist with monitoring and overseeing the SNF / LTC units to assure state and federal regulations are followed.
  • Ensure utilization of interdisciplinary rehab team process in the formulation of MDS / RAI's.
  • Observe direct nursing care, review documentation and make appropriate recommendations, assist with chart audits.
  • Provide clinical support and direct to appropriate resource materials.
  • Communicate well with clients, their significant others and various other professionals to facilitate high quality client care.
  • Ensure planning and implementation of appropriate nursing assessment and care programs.
  • Assist staff, as necessary, by administering direct client care according to ANA standards and hospital nursing procedures and standards.
