Facilitate and maintain relationships between the Administrative Entity, Support Coordination Organization, Day Programs, and Other Support Services. Avoid allowing personal beliefs and values to influence dealings with individuals receiving services.
Service Delivery/ Responsibilities
- The ODP Program Specialist is responsible for overseeing the creation, implementation, and maintenance of Individual Service Plans (ISPs) for residential group homes within the Intellectual and Development Disabilities (IDD) program.
- This role focuses on ensuring that services are delivered according to consumer preferences while training Direct Support Professionals (DSPs) to meet health safety needs effectively
- The specialist ensures quality care and facilitates service integration to enhance the quality of life for IDD consumers
- Oversee the creation, implementation, and maintenance of Individual Service Plans (ISPs) for residential group homes
- Ensure that Direct Support Professionals (DSPs) are properly trained to meet the health and safety needs of consumer as per their ISPs
- Facilitate the integration of services to improve the quality of life for IDD consumers
- Develop and oversee protocol for ISP processes to ensure compliance with regulatory changes
- Coordinate and assist in completing assessments, including the need for supervision
- Create and review ISPs with consumers at ISP meetings, allowing for adjustments as necessary
- Ensure ISPs are culturally appropriate and, and include consumer preferences and health risk screenings
- Oversee and facilitate regular ISP reviews, updates, and revisions
- Attend and actively participate in critical revision meetings
- Serve as an ISP team lead, implementing ISPs and training residential staff as needed
- Perform other function as deemed appropriate by the management team.
Supervision Received:
- Receives direct supervision from the Executive Director and / or Team Supervisor.
Supervision Exercised:
- May provide supervision to staff as directed by Associate Director and/ or Team Leader.
Qualifications:
- A bachelor’s degree from accredited college or university and 2 years of work experience working directly with person with disabilities.
- Experienced with HHA Exchange database, State Waiver Authorization & HIPPA.
- Knowledge of general healthcare staffing requirements. Must be a critical thinker and work with a sense of urgency.
- Ability to listen and communicate clearly, fluently, and diplomatically orally and writing.
- Ability to remain flexible, resilient, calm and maintain a sense of humor; and a well -groomed professional image.
- Ability to work independently and be a Team player, excellent verbal and communication skills.
- Strong computer skills are required, as well as Internet research abilities.
- Adaptable in difference situations and demonstrate a strong commitment to client service excellence.
- Able to multi-task and work independently.
- Must have experience using telephones with multiple lines, multi-task on several calls and maintain professionalism and patience with a fast-paced environment.
- Must be available to travel 30% of the time. Vehicle will be provided if or when needed.
- Must possess a visit driver’s license with an acceptable driving record.
Scope:
- Must be available for local travel.
- Frequent talking and listening.
- Must maintain frequent understanding and updates with HHA Exchange requirements, MCO’s, ODP (Office of Developmental Programs), OLTL (Office of Long Term Living), and State regulations.
- Participation in on-call coverages after 5PM weekdays and weekend.
- May be required to report to work during emergencies including inclement weather.
- May require crisis intervention for emergency situations.
- Require lifting and/ or carrying and the ability to fold, kneel, stack, stoop and bend which may include direct client interaction.
Job Type: Full-time
Pay: $20.00 - $23.50 per hour
