JOB DESCRIPTION:
The role of the Case Manager is to provide clinical services intended to aid individuals in establishing and maintaining individual recovery from addiction and mental illness. The Case Manager will help people in recovery to improve their quality of life through connecting them to social, spiritual, and peer supports.
JOB REQUIREMENTS:
Education:
- High school diploma or equivalent required
- 2 years’ experience in related field preferred
- Bachelor’s Degree Preferred
Experience: Experience in a behavioral health field preferred
Certification:
- Healthcare Provider CPR
- Level 1 Fingerprint Clearance Card and TB
- Handle with Care within the first 90 days of hire and annually thereafter
KNOWLEDGE/SKILLS/EXPERIENCE:
- Strong verbal and written communication skills in English
- Knowledge of community/recovery resources
- Ability to work both as a team member and independently
- Understanding of relapse prevention and harm reduction techniques
- Strong interpersonal skills
- Crisis management skills
DUTIES/RESPONSIBILITIES:
- Assists clients in developing an Individual Recovery Action Plan
- Helps clients navigate recovery support systems and access resources including health and wellness, housing, employment, and other professional and non-professional services.
- Assists clients with applying for benefits.
- Collaboratively completes service plans and ASAM.
- Coordinates after care services and assists client in transitioning back to the community.
- Acts as a supportive member of the clinical team and assists clients in managing day-to-day issues.
- Interfaces with DCS, court and referrals for behavioral health long term services.
- Brings the concept of recovery and support to the clinical treatment team.
- Assists the client in discharge planning and coordination of care.
- Helps clients to modify attitudes and patterns of behavior by increasing understanding of self, personal problems and client’s part in creating them.
- Educates and supports clients in implementing a range of relapse prevention and harm-reduction strategies.
- Works closely with the multidisciplinary team to ensure client needs are met.
- Actively participates in weekly supervision with Clinical Director and/or Assistant Clinical Director.
- Serves as a liaison between administration, staff, clients and volunteers.
- Other duties as assigned.
