Job Title: Member Outreach Specialist
We're looking for the next generation of healthcare leaders to join our team at AmeriHealth Caritas. As a Member Outreach Specialist, you will play a vital role in helping us build healthier communities.

Key Responsibilities:
- Establish connections with high-risk members through home visits and outreach calls
- Exercise judgment and discretion in coordinating care by handling basic follow-up tasks and collaborating with care management teams and community-based providers
- Inform members of required actions to fulfill HEDIS, EPSDT, and Plan requirements, and encourage compliance
- Assist in addressing members' barriers to care and foster relationships with medical and dental providers
Community Education:
- Develop and maintain relationships with community organizations via phone, mail, email, or personal visits
- Collaborate with internal departments and external agencies to obtain data for health education/promotion programs
Requirements:
- Associate's Degree in Healthcare, Human Services, or related field, or equivalent work experience
- Bachelor's Degree preferred
- Minimum 2 years of healthcare experience in an office/clinic/hospital setting or human services
- Comprehensive knowledge of Microsoft Office Suite and database management systems
- Community-based care experience preferred
- Previous experience presenting health education materials
