Community Health Navigator/Prevention Specialist
Monday to Friday 8:00am – 5:00pm (no weekends, nights, or major holidays)
Full-time (40/hour per week)
Locations available: We offer in-person work at our Knoxville locations, as well as the option of hybrid work for those who need more flexibility.

Key Responsibilities:
- Build relationships within the community, especially with underserved or vulnerable populations.
- Assist patients in accessing healthcare services, social services, and community resources, such as housing, food assistance, mental health services, and transportation.
- Provide health education on topics such as preventive care, nutrition, exercise, and chronic disease management.
- Work closely with healthcare providers, social workers, community organizations, and public heath agencies to coordinate services for patients.
Qualifications:
- Bachelor's degree in Social Work (BSW), Psychology, Sociology, or a related field.
- Previous experience in community outreach, case management, social services, or a healthcare setting is preferred.
- Ability to work with individuals from diverse cultural, linguistic, and socioeconomic backgrounds.
