FIELD-BASED REMOTE TO MICHIGAN - MACOMB & WAYNE COUNTY RESIDENTS ONLY
Responsibilities:;
- Serve as the primary liaison for targeted member populations, delivering direct support and coordinating follow-up care.
- Conduct proactive outreach through phone calls, emails, and in-person visits to educate and guide members on healthcare compliance, including HEDIS (Healthcare Effectiveness Data and Information Set) and EPSDT (Early and Periodic Screening, Diagnostic, and Treatment) requirements.
- Build and sustain strategic partnerships with community organizations, healthcare providers, and internal teams to enhance member access to care.
- Deliver impactful health education to members and representatives, ensuring adherence to Medicaid policies and managed care protocols.
- Maintain precise documentation and reporting to uphold regulatory compliance and program effectiveness.
- Engage with members in home and community-based settings, ensuring accessibility and personalized support.
- Associate’s degree in Healthcare, Human Services, or a related field required; Bachelor’s degree preferred.
- Minimum of two years of professional progressive experience in healthcare, human services, or a clinical setting, with a proven track record in community engagement.
- Strong knowledge of medical terminology, disease management, behavioral health, and Medicaid regulations.
- Exceptional communication, organizational, and problem-solving skills with the ability to build trust across diverse populations.
- Proficiency in;Teams, Database Management Systems, and Microsoft Office Suite, including PowerPoint, Outlook, and Excel;(highly desired).;
- Willing and able to travel up to 50%, conducting home visits and community-based meetings with members.
- Must have a valid driver’s license, current auto insurance, and reliable access to a personal vehicle for work-related travel.
