We are seeking a skilled professional to oversee the operational functions of our Long-Term Services and Supports (LTSS) Service Coordination team.
Responsibilities
- Develop and implement effective processes to support care delivery, ensuring quality metrics are met.
- Serve as a subject matter expert for LTSS training and coordinate benefits for all LTSS Participants.
- Maintain operational processes, policies, and procedures to support care delivery.
Requirements
- Bachelor's degree required.
- Pennsylvania licensed RN, LSW, or LPC required.
- 1-3 years of leadership and/or supervisory experience.
- 3 years working in social service or healthcare-related field.
