Responsibilities:
Under the supervision of the Manager, Corporate Population Health Programs, the Supervisor is responsible for the daily oversight of the Complex Care Management and/or Care Coordination unit providing clinical direction, organization, and monitoring of all care coordination services. Responsible for direct supervisory oversight of staff conducting care coordination/management screening and/or other supportive functions. Demonstrates ability to act as a resource in addressing clinical and technical issues. Demonstrates and maintains a current knowledge of State and Federal requirements, licensing and regulating agency requirements, e.g., NCQA standards. Provides unit and departmental leadership. Responsible for reporting oversight. Assists in carrying out the plans and daily operations of the Complex Care Management unit and/or Care Coordination unit and other duties as assigned.

Education/Experience:
- Registered Nurse with BSN or LMSW/LCSW with Behavioral Health background.
- An unrestricted RN/LMSW or LCSW license to practice nursing in the Plan’s state and any other state in which he/she works.
- 3 to 5 years Clinical practice experience and must be able to practice care management within the scope of the licensure based on the standards of the discipline.
- Must have 2 years supervisory experience.
Other Skills:
- Proficiency with Microsoft. Office Suite (Word, Excel, PowerPoint). Access is a plus.
- Consistent word processing speed and accuracy of 50 or more words per minute.
