Job Summary
This position serves as the first point of contact for patients and physicians, responsible for reviewing and scheduling deliveries for home infusion medication and nursing services. It includes clerical duties related to managing care, equipment, and documentation necessary for compliance. Additionally, the role acts as a reference point for any clinical interventions that may require the involvement of a pharmacist.

- Location: On-Site (Laguna Hills, CA)
- Hours: Monday-Friday 7:00 AM- 3:30 PM
Duties and Responsibilities
- Patient Assessment: Collecting comprehensive assessments to determine patient eligibility and suitability for home infusion therapy, including weekly evaluations to identify any changes in patient care needs.
- Scheduling Deliveries: Manage initial and refill delivery schedules, ensuring an organized and up-to-date calendar to prevent disruptions in care. Complete assessments based on pharmacist recommendations and ensure follow-up deliveries and progress notes are maintained.
- Patient Education: Provide clear, thorough information to patients and their families regarding the home infusion therapy process, care transition, and medication delivery procedures.
- Documentation and Reporting: Maintain accurate, up-to-date electronic patient records, tracking progress throughout the service. Follow up to ensure all necessary patient documents are signed and collected.
- Quality Assurance: Ensure strict compliance with healthcare regulations, standards, and organizational policies. Implement quality assurance measures to monitor patient satisfaction, address issues promptly, and continuously improve the patient care process.
- Collaboration: Work closely with pharmacy and clinical teams to facilitate accurate ordering and timely delivery of medications, supplies, and equipment for home infusion therapy.
- Communication: Manage patient calls that require escalation to a pharmacist for clinical interventions, such as adverse events, changes in patient status, medication counseling, or drug interaction checks.
- Billing: Collect and verify updated insurance benefits for refill patients to ensure accurate billing.

Education and Experience Requirements
- High school diploma or equivalent.
- At least two years of related experience in healthcare.
Preferred Qualifications
- Pharmacy Technician license is highly preferred.
- 1 years of experience with TPN (Total Parenteral Nutrition).
