Job Summary:
The Case Manager is primarily responsible to build a support system for families and connect them with people and programs in the community. The Case Manager is a resource for new visitors to prepare families for permanent housing and ensure they have access to a support network once housed. They will conduct assessments, address barriers to housing, coordinate with housing search staff, and connect families with community resources. Additionally, the Case Manager will provide mental health services, crisis support, and counseling to both shelter residents and personnel.
Duties/Responsibilities:
- Case Management and Support Services
- Assess the strengths, needs, and barriers to housing for each family, collaborating with housing search staff.
- Assist families in resolving or mitigating tenancy-screening barriers, such as securing identification documents.
- Support families in completing activities outlined in the Rehousing and Stabilization Plan.
- Connect families with healthcare services, including community health clinics.
- Secure connections to job training and job placement services for eligible household members.
- Help families access benefits such as SNAP, TAFDC, and SSI.
- Work with parents to address educational and health needs of children, enrolling them in appropriate services.
- Document case management efforts and meetings using the designated case management system.
- Community Resource Coordination
- Connect families with appropriate ancillary services, such as MassHealth, SNAP/TAFDC, childcare, and job training programs.
- Maintain effective communication and coordination with Housing Search Workers to determine rehousing plans for families.
- Assess the resources needed by families to support their rehousing goals.
- Continuously evaluate and update available community resources to ensure families have access to comprehensive support.
- Coordinate between welcome center and new visitor shelter.
- Mental Health Services
- Designate a secluded area within the shelter to provide individuals with mental health services.
- Assess the mental health needs of shelter residents and provide crisis support as necessary.
- Collaborate with the Welcome Center to offer additional mental health support if required.
- Coordinate activities for children that help them express their feelings about the displacement or disaster.

- Proficiency in technology including Microsoft Office Suite including Word, Excel, and Office.
- Excellent communication and interpersonal skills to work effectively with individuals from diverse backgrounds.
- Ability to handle sensitive information with confidentiality and maintain professional boundaries
- Strong assessment and problem-solving skills to address the needs and barriers of individuals and families
- Ability to maintain accurate and detailed case notes using designated case management software.
Education & Experience:
- Bachelor's degree in social work, psychology, counseling, or a related field. Master's degree preferred.
- Proven experience in case management, preferably in a diverse population such as a shelter or social services setting.
- Knowledge of housing search processes, tenancy-screening barriers, and community resources.
- Familiarity with health and social service programs, such as MassHealth, SNAP/TAFDC, and job training initiatives..
- Knowledge of child and family services, educational systems, and resources.
- Demonstrated understanding of mental health support and crisis intervention techniques..

- Written and spoken fluency in English
- Fluency in Haitian Creole and multiple languages is a plus.
