Description
Full time position in either Littleton NH or Woodsville NH. The ACHS Medical Information Specialist is responsible for entering clinical and other data into the Electronic Health Record System (EHR) and creating/distributing scheduled reports from the system. The source for this data may be paper based, or from other electronic sources (Hospital data system, dictated notes, Emails, Etc). The document management tools, IndxLogic and Docutrak, are used to manage the workflow.
Requirements
- Enters alphabetic, numeric, or symbolic data from source documents into Electronic Health Record System via specific templates or flowsheets.
- Compares data entered with source documents, or re-enters data in verification format within the EHR to detect errors of transcription.
- Collects, compiles, sorts and verifies accuracy of data to appropriate charts, templates, or flowsheets. This may include the daily mail and data collection from various sources.
- Use the IndxLogic and Docutrak application to enter the data into appropriate observations terms, charts and templates.
- Utilize tools such as Email, Citrix, Cottage Hospital HMS or DHConnect along with other technology-based resources to gather and transfer data into the appropriate patient chart.
- May transcribe patient notes and letters dictated by clinical providers. Goal is to transcribe notes within 24 hours of dictation.
- Assist with clinical tracking by reviewing flow sheets to assure that correspondence has correct clinical notes.
- Proactively improve workflow to minimize duplicate records and other errors, with a focus on constant, incremental improvement.
- Supports other data entry staff by addressing issues or questions that may arise. This staff includes internal staff members and data entry clerks not directly affiliated with ACHS.
- Train other data entry staff as need warrants.
- Manual data entry to meet State and Federal requirements
- Follow HIPAA guidelines to process Release of Information requests
- May be required to maintain documentation and master book for reference and training purposes.
- Other duties, hours and ACHS site placement may be assigned.
