Job Description:
As a full-time RN care coordinator or LPN, you will devote more time to quality end-of-life bedside care, leveraging technology to improve patient outcomes, providing education to patients and families, maximizing hospice medical skills, coordinating plan of care development, and collaborating as part of a dynamic interdisciplinary team.
