GENERAL PURPOSE:
Under the supervision of the HIV Program Manager, the Outreach & Prevention Care Coordinator is responsible for providing prevention and early intervention services to a diverse client population. This position is responsible for providing coordinated care for patients for various clinical screenings and testing. Performs HIV, Hepatitis C antibody testing, various STI screenings, and referral services.
MAIN RESPONSIBILITIES AND DUTIES:
- Provides direct, face-to-face services in clinics, outreach events, and various locations within Ampla Health’s six-county area. Travel using a company vehicle is required.
- Demonstrates flexibility and openness to new information, adjusting behavior and work methods as conditions change.
- Refers clients to internal and external resources, including pharmacy, medical care, and support services, based on treatment needs.
- Works independently and collaboratively with the Ryan White team, maintaining excellent communication with patients and team members.
- Utilizes strong verbal and written communication skills to enhance patient understanding of health needs while ensuring confidentiality and respect.
- Conducts continuous outreach and engagement activities, coordinating schedules with the outreach team.
- Availability for occasional weekend and evening shifts as required.
- Utilizes databases to collect, analyze, and report data to optimize efficiency and quality.
- Attend training, meetings, and in-services as required, including the State Office of AIDS, HIV Tests, and Counselor certification program if needed.
- Treats patients with empathy and professionalism, especially those dealing with drug use, mental illness, or chronic health challenges.
- Adheres strictly to procedures regarding confidentiality, release of information, and data security.
- Conducts weekly, monthly, and quarterly reports, ensures data entry, and provides monthly patient follow-ups.
- Runs control tests, and maintains process, procedural, program, and inventory logs accurately.
- Ensure proper collection, labeling, and documentation of specimens.
- Provides navigation services for HIV and PrEP.
- Other Duties as assigned by the supervisor.

- High School Diploma required.
- Completion of Medical Assistant or Phlebotomy certification required.

- Proficient in Microsoft Word, Excel, Outlook, and PowerPoint.
- Able to interact professionally in person and over the phone, using culturally competent and trauma-informed approaches when communicating with patients for appointments and follow-ups.
- Proven ability to promote testing and screenings while consistently meeting quotas.
- Capable of developing and managing daily testing and screening schedules.
- Attuned to the cultural needs of the diverse patient population served by the organization.
- Demonstrates a thorough understanding of Ampla Health's clinic structure, standards, procedures, and protocols.
- Familiar with the challenges faced by people of color, immigrants, high-risk youth, low-income families, drug users, individuals in the justice system, the homeless, and those with chronic illnesses or mental health issues.
COMMUNICATION SKILLS:
- Must maintain neat and legible handwriting.
- Must interact courteously and calmly with patients.
- Strong communication skills when engaging with the public.
- Exceptional interpersonal, written, and verbal communication skills.
WORKING CONDITIONS & PHYSICAL REQUIREMENTS: Generally, a comfortable office setting with patient interaction.
- Must be in good health and able to lift up to 40 lbs.
- Clear hearing and communication skills for phone and in-person interactions.
- Able to operate standard office equipment (computer, phone, fax, copier, etc.).
- Must perform physical tasks such as reaching, bending, squatting, standing, walking up to a mile, and climbing stairs.
- Responsible for fueling and maintaining company vehicles. If a company vehicle is unavailable, a reliable personal vehicle with valid insurance, a California driver’s license, and a clean driving record is required.
- Maintains a safe driving record, ensuring reliability and safety during work-related travel.
- May be exposed to bloodborne pathogens.
