Description

GENERAL PURPOSE:

Under the supervision of the HIV Program Manager, the Outreach & Prevention Care Coordinator is responsible for providing prevention and early intervention services to a diverse client population. This position is responsible for providing coordinated care for patients for various clinical screenings and testing. Performs HIV, Hepatitis C antibody testing, various STI screenings, and referral services.

MAIN RESPONSIBILITIES AND DUTIES:

  1. Provides direct, face-to-face services in clinics, outreach events, and various locations within Ampla Health’s six-county area. Travel using a company vehicle is required.
  2. Demonstrates flexibility and openness to new information, adjusting behavior and work methods as conditions change.
  3. Refers clients to internal and external resources, including pharmacy, medical care, and support services, based on treatment needs.
  4. Works independently and collaboratively with the Ryan White team, maintaining excellent communication with patients and team members.
  5. Utilizes strong verbal and written communication skills to enhance patient understanding of health needs while ensuring confidentiality and respect.
  6. Conducts continuous outreach and engagement activities, coordinating schedules with the outreach team.
  7. Availability for occasional weekend and evening shifts as required.
  8. Utilizes databases to collect, analyze, and report data to optimize efficiency and quality.
  9. Attend training, meetings, and in-services as required, including the State Office of AIDS, HIV Tests, and Counselor certification program if needed.
  10. Treats patients with empathy and professionalism, especially those dealing with drug use, mental illness, or chronic health challenges.
  11. Adheres strictly to procedures regarding confidentiality, release of information, and data security.
  12. Conducts weekly, monthly, and quarterly reports, ensures data entry, and provides monthly patient follow-ups.
  13. Runs control tests, and maintains process, procedural, program, and inventory logs accurately.
  14. Ensure proper collection, labeling, and documentation of specimens.
  15. Provides navigation services for HIV and PrEP.
  16. Other Duties as assigned by the supervisor.

PROFESSIONAL KNOWLEDGE, SKILLS & ABILITIES:

  1. High School Diploma required.
  2. Completion of Medical Assistant or Phlebotomy certification required.
  3. Strong conflict resolution skills with the ability to solve problems through a collaborative, team-oriented approach.
  4. Proficient in Microsoft Word, Excel, Outlook, and PowerPoint.
  5. Able to interact professionally in person and over the phone, using culturally competent and trauma-informed approaches when communicating with patients for appointments and follow-ups.
  6. Proven ability to promote testing and screenings while consistently meeting quotas.
  7. Capable of developing and managing daily testing and screening schedules.
  8. Attuned to the cultural needs of the diverse patient population served by the organization.
  9. Demonstrates a thorough understanding of Ampla Health's clinic structure, standards, procedures, and protocols.
  10. Familiar with the challenges faced by people of color, immigrants, high-risk youth, low-income families, drug users, individuals in the justice system, the homeless, and those with chronic illnesses or mental health issues.

COMMUNICATION SKILLS:

  1. Must maintain neat and legible handwriting.
  2. Must interact courteously and calmly with patients.
  3. Strong communication skills when engaging with the public.
  4. Exceptional interpersonal, written, and verbal communication skills.

WORKING CONDITIONS & PHYSICAL REQUIREMENTS: Generally, a comfortable office setting with patient interaction.

  1. Must be in good health and able to lift up to 40 lbs.
  2. Clear hearing and communication skills for phone and in-person interactions.
  3. Able to operate standard office equipment (computer, phone, fax, copier, etc.).
  4. Must perform physical tasks such as reaching, bending, squatting, standing, walking up to a mile, and climbing stairs.
  5. Responsible for fueling and maintaining company vehicles. If a company vehicle is unavailable, a reliable personal vehicle with valid insurance, a California driver’s license, and a clean driving record is required.
  6. Maintains a safe driving record, ensuring reliability and safety during work-related travel.
  7. May be exposed to bloodborne pathogens.
