Basic Functions: FSC is provided through the NM FIT Early Intervention program. FSC services are provided, as designated in the Individualized Family Service Plan (IFSP) and performed by a designated individual to assist and enable the families of children from birth through age three (3) years of age to access and receive early intervention services. The responsibilities of the FSC include acting as the single point of contact for: coordinating, facilitating and monitoring the delivery of services to ensure that services are provided in a timely manner; coordinating services across agency lines; assisting parents in gaining access to, and coordinating the provision of, early intervention (EI) services and other services as identified on the IFSP; explaining to families about the early intervention and their procedural safeguards; gathering information from the family regarding their concerns, priorities and resources; coordinating the evaluation and assessment activities; facilitating the determination of the child’s eligibility; referring the family to providers for needed services and supports; scheduling appointments for IFSP services for the child and their family; helping families plan and prepare for their IFSP meeting; organizing, facilitating and participating in IFSP meetings; arranging for and coordinating all services listed on the IFSP; conducting follow-up activities to determine that appropriate services are being provided; coordinating funding sources for services provided under the IFSP; monitoring the delivery of the services listed on the IFSP; facilitating periodic reviews of the IFSP; and ensuring that a transition plan is developed at the appropriate time.
Scope: FSC is responsible for providing family service coordination to families/children receiving EI services in accordance with the EI process as defined by the NM FIT Program regulations and standards, and for participating in business operating duties.
Minimum Qualifications:
- Minimum Education requirements:
- A bachelor’s degree in social work, counseling, psychology, nursing, special education, early childhood education, or any other closely related field.
- A bachelor’s degree in another field plus two (2) years of direct experience in serving individuals with disabilities and/or families.
Training and Supervision Requirements:
- Minimum Training and Supervision requirements:
- Training for family service coordination is in accordance with NM FIT Program standards within one (1) year of hire.
- Completion of the web-based family service coordination training every three (3) years.
- Receive reflective supervision at least once (1) every month.
Responsibilities and Objectives:

- May be asked to coordinate the intake, evaluations and assessments, eligibility, and development of the IFSP.
- Facilitates and participates in multidisciplinary meetings to review evaluation results and integrate findings with other disciplines.
- Coordinates and monitors the delivery of services (including subcontractors and providers from other agencies).
- Participates in team discussions in an articulate, positive, and productive manner.
- Maintains equipment in proper working order. Performs routine repairs.
- Instructs staff, as assigned, in principles used in family service coordination.
- Completes all documentation in a timely manner.
- Attends departmental/business staff development activities.
- Maintains proficiency in family service coordination by attending workshops, conferences, and trainings.
- Attends all mandatory in-services and participates in Company committees as requested by supervisor.
- May represent the Company and FSC department while participating in community outreach events, activities, meetings, trainings, etc.
- Responds to emergency situations in a competent manner.
- Adheres to established Company policies and procedures.
- Assists in quality management, research, public relations, and program development projects when appropriate.
- Will have assigned duties within Company’s Professional Development Plan
- May be assigned to be an FSC supervisor for other FSCs.
- Performs other duties as assigned.

- Duties may involve being on one’s feet for extended periods.
- Duties may involve traveling to and from areas within the southwest region of New Mexico.
- Duties may involve exposure to hazardous conditions, contagious diseases, and chemicals to repair/adjust patient equipment
