Position Title: Director of Case Management
Job Summary:
The Director of Case Management oversees all departmental activities, ensuring alignment with organizational policies and regulatory requirements. This role requires a dynamic leader to coordinate patient care and discharge planning processes while optimizing clinical resource utilization. The director will manage daily hospital case management operations, drive interdisciplinary collaboration, and maintain strong relationships with key stakeholders.
Responsibilities:
- Lead and evaluate departmental operations, staffing, and resource allocation to meet productivity and quality goals.
- Oversee care coordination, discharge planning, and compliance with established protocols.
- Collaborate with the CFO and Division CM Leader to achieve performance objectives.
- Implement sustainable performance improvement initiatives through data analysis and operational review.
- Build and maintain strategic relationships with key stakeholders, including the CMO, CNO, and ancillary service leaders.
- Ensure adherence to the organization's Code of Conduct and Mission and Value Statement.
Qualifications:
- At least 2 years of recent experience as a Director of Case Management in a large acute care facility.
- Registered Nurse (RN) licensure required; LCSW certification not accepted.
- Bachelor's degree required; Master’s degree in Nursing, Health Administration, or Business Administration preferred.
- Strong executive leadership experience, including working with C-suite executives.
- Proven ability to manage complex case management environments and lead interdisciplinary teams effectively.
Compensation & Benefits:
- Salary: $110,000–$130,000 (based on experience).
- $10,000 sign-on bonus.
- Relocation assistance available (case by case).
- 15% performance-based bonus target.
