Job Summary:
The Manager of Case Management is responsible for promoting patient-centered care by overseeing hospital-based case management services. This role includes the coordination, supervision, and administrative oversight of the case management team to ensure effective patient care planning, utilization management, and compliance with hospital policies and regulatory guidelines. The Manager serves as a clinical resource, mentor, and leader for the case management staff, ensuring optimal patient outcomes and resource utilization.
Key Responsibilities:
- Supervise and provide leadership to the case management team, ensuring competency in care coordination practices.
- Serve as a role model and clinical resource for case management staff.
- Monitor the quality of interactions, documentation, and assessments of case managers through audits and staff shadowing.
- Collaborate with physicians, nurses, and interdisciplinary care teams to facilitate timely and appropriate patient management.
- Oversee staff scheduling, training, and competency development.
- Participate in clinical performance improvement initiatives to enhance patient care and efficiency.
- Ensure adherence to regulatory requirements, hospital policies, and quality improvement initiatives.
- Advocate for patients' rights, cultural values, and healthcare needs.
- Maintain departmental reports and support budgetary management without compromising care quality.
- Assume departmental leadership responsibilities in the absence of the Case Management Director.
Qualifications:
- Active Registered Nurse (RN) license (Missouri or Compact State).
- Preferred: At least one year of case management experience.
- Considered: RN leadership experience (e.g., Charge Nurse, Supervisor) without prior case management experience.
- Strong communication, problem-solving, and leadership skills.
Compensation & Benefits:
- Salary Range: $83,000 - $114,000 (based on management experience).
