Purpose
- To oversee day support professional staff to ensure they provide quality daily hands-on care and supervision of clients
- To assist the Director of Program Development with updating and the implementation of the Individual Habilitation Plans and Essential Life Plans
- Responsible for building a staffing team and supervision of Day Habilitation Support Professionals assigned to their program(s), the evaluation of staff, and the recommendation and implementation of personnel actions when necessary
- To ensure the provision of services are delivered in a safe, secure, and dependable manner. The PM will support and assist in areas necessary for service recipients to achieve full social inclusion, independence, personal and economic well-being as approved by the Individual Service Plan (ISP)
Duties & Responsibilities
- Facilitate and ensure full social inclusion, independence, and personal and economic well-being of service recipients. The PM ensures that service recipients are progressing towards his/her personally defined outcomes as indicated in the Individual Service Plan (ISP)
- Responsible for notifying appropriate personnel admissions decisions, whether the admission was accepted or denied
- Assist in the development and coordination of weekly and monthly activity schedules and community-based activities
- Train, maintain and quality assure all required documentation for operating the day habilitation program
- Ensure that all required documentation for billing purposes are entered into Day Hab Program’s documentation and billing software
- Assist in the training, guiding and directing of Program Supervisors with their responsibilities. Ensure they are receiving the proper guidance necessary to perform all job duties at full capacity
- Train and guide Program Supervisors to properly train their direct care staff, and to ensure all staff in compliance with all state, federal and DDD regulations
- Ensure that basic facilities, such as water and heating, are well-maintained
- Ensure facility meets state, federal and DDD regulations and environmental, health, and security
- standards
- Inspect the locations’ structures to determine the need for repairs or renovations
- Review utilities consumption and strive to minimize costs
- Conduct and coordinate all community outings; including ensuring client to staff ratio is appropriate, ensuring all individuals are properly transported, etc
- Perform all other relevant duties as assigned by the Executive Director or designee
