Position Summary
The Service Coordinator (Bilingual Hindi/Bengali) manages a caseload of 25 patients, ensuring they receive the resources and support necessary to remain in their homes. This role combines task-based responsibilities, such as coordinating supplies and transportation, with problem-solving to address patient concerns and ensure program retention.
Key Responsibilities
- Maintain regular communication with patients to address needs.
- Prevent discharges by troubleshooting and resolving issues.
- Coordinate quarterly home visits and complete timely documentation.
- Use internal systems like Bolt, HHAX, and Smartsheet to manage
- patient needs.
- Facilitate patient access to resources such as supplies and transportation.
Requirements
- Must reside in New York
- 2-3 weeks in-person training required (on-site in Williamsburg)
- Bachelor's Degree required
- Must speak English and Hindi / Bengali Fluently!!!
- Must be wiling to conduct home visits throughout the 5 boroughs of New York

Homecare Service Coordinator Schedule
- Initial training ( 2-3 weeks ) is in person and then transition to remote
- Home visits to clients throughout the 5 boroughs of New York
- On call remotely Monday to Friday 9am-5pm for patient assistance
