The Case Manager position provides case management support to youth ages 16-21 living in our transitional living residences and on call support to the residents and the residential advisors in the case of unusual incidents and emergencies. This position will also provide individual, family, and group counseling services and will prepare case plans that maximize the youth’s strengths while providing support for the youth’s needs and monitor their success to meet goals.
The ideal candidate would possess:
- Strong communication skills and the ability to collaborate with the team to ensure the quality of service to the youth.
- The ability to maintain self-awareness as a youth development worker and set limits to maintain a helper role.
- Respect for diversity and the differences among the youth, their families, and the communities we serve.
- An understanding of youth development.
- The ability to create and sustain relationships with the youth that motivates and empowers positive decision making.
- The capacity to work positively and effectively communicate with the community and other agencies and groups on behalf of the youth we serve.
This position requires a bachelor’s degree in counseling or social work and two years of related experience.
A valid driver’s license is required.
A successful candidate must pass a level 2 background screen.
