Job Summary: Provides case management services at a mental health practice to assist and support individuals in gaining access to needed medical, social, educational and other services essential to meeting basic human needs. Services may include interactions with family members, other individuals or entities.
Supervised by: Clinical Director
Duties/Responsibilities Under the direction of a qualified supervisor, and consistent with a client’s Individual Treatment / Service Plan, provide services that support the client’s goals and objectives and improves the overall functioning of the client.
- Transport clients to and from appointments/groups. (Company provided vehicle)
- Coordinate client assessments, treatment planning and crisis intervention services.
- Provide training and facilitate linkages for the use of community resources.
- Monitor service delivery.
- Obtain or assist individuals in obtaining necessary services, for example, financial assistance, housing assistance, food, clothing, medical services, educational services, vocational services, recreational services, etc.
- Assist individuals in becoming involved with self-help support groups.
- Assist individuals in increasing social support networks with family members, friends, and/or organizations.
- Document progress notes clearly and thoroughly in the electronic health record within agency timelines.
- Strive to meet the agency’s productivity requirements of the position.
- Collaborate and communicate with other staff members, including therapists, counselors and other team members to ensure high-quality treatment and coordination of care.
- Attend and participate actively in multi-disciplinary team meetings.

- High School diploma or G.E.D. certificate. Bachelor’s degree in human services field preferred.
- Minimum of one-year related experience required.
- Alternating periods of sitting, standing and moving while meeting needs of clients May travel by car in community to see clients
- Must have valid current Ohio driver’s license and carry current auto insurance compliant with Ohio requirements.
