The Opportunity
We are looking for a case coordinator AND office assistant.
The Case Coordinator will be responsible for coordinating the client's care provided by the certified aides, from the start to finish.
Essential Duties & Responsibilities
- Supervise staff and provide direction on all aspects of the program
- Match the clients with approriate CHHA
- Ensure that all required documentation is completed and accurate.
- Set-up the initial schedule for homecare services and coordinate all changes
- Complete any additional duties requested by the Executive Director.
Qualifications: Education: Bachelor’s degree in social work, psychology, sociology, human services, or related field preferred, but not required. Must have a valid driver’s license and reliable transportation. Must be a team player who is able to work independently as well as part of a team environment. Must be self-motivated with a strong sense of urgency. Must be able to work independently as well as part of a team environment. Must be highly organized with excellent attention to detail. Must be able to communicate effectively both verbally and in writing. Must be able to travel up to Toms River, NJ. Must be bilingual in English and Spanish. Must have experience as a Homecare Coordinator within Medicaid Agency for at least 2 years. Must know CAHC standards.
The Office Assistant will help with above duties as well as anything else that's needed to run a homecare agency.
Job Type: Full-time
Pay: $20.00 - $25.00 per hour
