Title : Clinical Social Worker, Lewiston - 72 Strawberry Ave
What Youll Do :
- Assesses and identify the clients holistic health care needs / concerns as part of a family and community.
- Assesses social / emotional factors that are related to the patients illness, need for care, response to treatment, and adjustment to care.
- Develops a plan of care under physician orders with realistic / measurable outcomes in collaboration with the client / family and health care team providers according to Organization protocols.
- Implement patient care plan activities based on treatment goals towards independent living for the patient / family.
- Serve as resource for staff education & training, participate in case conferences as needed.
