What are the responsibilities and job description for the HOSPICE AND PALLIATIVE CARE NURSE PRACTITIONER position at Androscoggin Home Healthcare + Hospice?
Title : Hospice & Palliative Care Nurse Practitioner, Lewiston
What Youll Do :
- Assist family meetings to clarify goals of care, code status and, when appropriate, transition to hospice.
- Assist with or facilitate family meetings to clarify goals of care, code status and when appropriate, transition to hospice.
- Coordinate care among both internal and external clinicians and health care providers.
- Provide on call services via phone for hospice patients, both in the community and at the hospice house.
- Participate in care coordination meetings as needed, including mornings ICU rounds, St. Marys oncology, hospice house and palliative care interdisciplinary team meetings.
- Act as a resource to staff, patients, family members and attending providers regarding pain and symptom management.
- For patients at the Hospice house, serve as attending or covering provider and is responsible for ordering medications, treatments, and testing as appropriate.
- Perform palliative care initial consultations and follow up visits in the community or acute care setting as requested by the primary care team.
- Provide care based on current clinical practice guidelines and evidence-based practice.
